Who where they and what did they do?
The Health and Social Care Change Agent Team (CAT) was set up in 2002 to help health and social care communities tackle the problem of patients staying in hospital longer than necessary. Since then the remit has grown and the CAT now provides advice and support on a wide range of issues that affect the care of older people including assessment, commissioning and intermediate care.
